General Registration

INSTRUCTIONS:

Registration for the conference is required. Please download and print out a copy of the form below. You may fill out the form by typing or neatly printing the requested information. Please fill out the form completely. If you are NOT registering for the entire conference, indicate on the registration form which days you plan to attend. Do not use this form if you are applying for a stipend, use the Stipend Registration form.

Please submit one (1) form per person attending. PERSONAL ASSISTANTS MUST REGISTER SEPARATELY, but may apply for stipends.

NO REFUNDS AFTER MAY 16, 2008.

Stipends Registration

PERSONAL ASSISTANTS MUST REGISTER SEPARATELY.

Stipends are limited and will be accepted on a first come, first serve basis. MegaConference partnering organizations have offered funds for stipends and scholarships, however, each organization has their own requirements for use of their funds. Determination of stipend funds will be made upon receipt of your Stipend Registration. You will receive confirmation of your Stipend Registration within 72 hours.

Special Accommodations?

Special accommodations will be provided upon request only. Requests for special accommodations MUST be received by April 30, 2008.

F.A.Q.

Q: Do I need to book my own room?

A: Yes! Unless you specify stipend money be used for lodging, you need to make your own reservations.

Q: Can I submit a purchase order for registration?

A: No. Sorry, we are not accepting purchase orders.
