• Stipends are limited and will only be applied to registration fees.
• This form should be filled out only if you are applying for a stipend.
• Please submit one (1) form per person requesting a stipend.
• Direct Support staff must register seperately, but may apply for stipends.
• Please print or type clearly- include complete information.
• All meals are included in your registration fees.

• NO REFUNDS AFTER SEPTEMBER 15, 2011

• All requests for special accommodations must be received no later than September 9, 2011.

• Hotel reservations must be made by each attendee NO LATER THAN September 5, 2011.
• Hotel lodging costs $114.00/night + 10% tax for a Deluxe King and $124.00/night + 10% tax for a Deluxe Double Queen. See MegaConference website for more information.
• Hotel reservations can be made by calling 1-877-540-7800 and you must mention the MegaConference to receive the conference rate.
